Dear Sir,

Acknowledgement is hereby made of your Freedom of Information (FOI)
request concerning the replacement of your lost NASSCORP ID card.

Kindly be informed that NASSCORP ID card  is free of charge,  but when
lost, a replacement fee of US$5.00 is paid before reissuance.

If you are ready, you can proceed to the head office of NASSCORP on 15th
Street & Payne Avenue, Sinkor, and contact the Registration Department for
the processing of your replacement ID card.

I hope this information will serve your purpose.
